Surgical Mistakes Continue Despite VA Initiative (CME/CE)

Despite efforts to reduce them, surgical mistakes involving the wrong patient, wrong procedure, wrong site, or wrong implant continue to occur within the operating room and elsewhere in hospitals, a Veterans Health Administration study found.
